C.A.I.M. Visitors Log Book

General Instructions:

A. The Visitor's Log is an official log available to retrieve all the necessary information.

B. All entries to be inserted as necessary using a ball pen.

C. The Visitors' Log is an official log to be kept on board for a period of 3 (three) after the last entry is made in the record.

D. If any information needs to be corrected, do not over write, cross with ball pen the necessary words and sign as necessary.
